Key facts

What is CBL-514?

CBL-514 is a first-in-class, injectable small-molecule compound that triggers apoptosis — programmed cell death — in fat cells. Developed by Caliway Biopharmaceuticals, it works by inhibiting the kinase DYRK1B, which raises Caspase-3 and the Bax/Bcl-2 ratio. It has completed multiple Phase 2 studies and is now in two FDA-cleared global Phase 3 trials for abdominal subcutaneous fat reduction, with results expected in 2027. It is not approved for human use.

Compound name CBL-514 (also written CBL514, CBL 514)
Compound class Small molecule (not a peptide)
Originator Caliway Biopharmaceuticals (Taiwan)
Molecular target DYRK1B inhibition → ↑ Caspase-3, ↑ Bax/Bcl-2 ratio
Primary research area Localized subcutaneous adipose reduction; also studied in cellulite, Dercum’s disease and lipoma models
Mechanism type Apoptosis (programmed cell death), as opposed to necrosis
Clinical stage Phase 3 — SUPREME-01 (CBL-0301, NCT07140939) and SUPREME-02 (CBL-0302)
Regulatory status Investigational. Not FDA approved. EMA orphan designation for Dercum’s disease.
Royal Peptides spec ≥99% HPLC · MS identity confirmed · 20 mg lyophilized · Lot RPCBL1

Mechanism of action

How CBL-514 works: fewer fat cells, not smaller ones

Most metabolic research compounds — including GLP-1 and GIP receptor agonists — shrink adipocytes by changing energy balance. CBL-514 is studied for a different endpoint: reducing adipocyte number in a localized area.

In preclinical and clinical work, CBL-514 has been shown to induce adipocyte apoptosis by inhibiting DYRK1B. Apoptotic cells shrink and fragment internally while keeping their membranes intact, which is why researchers distinguish it from necrotic agents that rupture cells and drive local inflammation.

Why researchers pair it with incretin models

Caliway has presented preclinical data in diet-induced obese rats combining CBL-514 with tirzepatide, reporting enhanced weight loss and reduced weight regain. A 120-subject Phase 2 combination study (CBL-0201WR) has been cleared by the FDA.

Published research

CBL-514 clinical research timeline

CBL-514 has one of the deepest data packages of any compound in the localized-fat category. Caliway reports that 10 completed clinical trials involving 544 subjects met their efficacy and safety endpoints.

A randomized Phase 2b trial of 108 adults with moderate-to-severe abdominal fat — up to four treatments, three weeks apart — was published in Aesthetic Surgery Journal in May 2026, using clinician- and patient-reported outcomes plus MRI.

In the separate Dercum’s disease Phase 2 study, CBL-514 met its primary pain-reduction endpoint (3.1 vs 1.9 points for placebo, p = 0.005), with no drug-related serious adverse events reported. Injection-site reactions were the most common tolerability finding and were graded mild to moderate.

2023FDA IND · Phase 2 (CBL-0202)

2024EMA orphan drug designation

2025Phase 2b success (CBL-0204, CBL-0205)

Both met primary endpoints. FDA clears SUPREME-01 (CBL-0301) Phase 3 in July.

Laboratory handling

Storage & handling

Lyophilized CBL-514 should be stored at −20 °C in its sealed vial, protected from light and humidity. Allow the vial to equilibrate to room temperature before opening to prevent condensation on the powder.

Reconstitution solvent and concentration should follow your laboratory’s validated protocol and the solubility information on the lot COA. Aliquot reconstituted material to avoid repeated freeze–thaw cycles.

FAQ

CBL-514 frequently asked questions

What is CBL-514?

CBL-514 is a first-in-class small-molecule compound developed by Caliway Biopharmaceuticals that induces apoptosis (programmed cell death) in fat cells. Its proposed mechanism is inhibition of DYRK1B, which raises Caspase-3 expression and the Bax/Bcl-2 ratio. It is currently in Phase 3 clinical trials for abdominal subcutaneous fat reduction.

Is CBL-514 a peptide?

No. Despite often being discussed alongside research peptides, CBL-514 is a small molecule, not a peptide chain. This affects its handling, solubility and analytical testing.

Is CBL-514 FDA approved?

No. CBL-514 is an investigational compound. The FDA cleared two pivotal Phase 3 studies (SUPREME-01 / CBL-0301 and SUPREME-02 / CBL-0302), with results expected in 2027. It is not approved for any human use. Royal Peptides supplies it strictly for laboratory research.

How does CBL-514 differ from deoxycholic acid (ATX-101)?

Deoxycholic acid is a detergent-like agent that lyses fat cells by disrupting membranes (necrosis), which is associated with inflammation. CBL-514 is reported to work through apoptosis, a controlled cell-death pathway, which researchers are studying because it may spare surrounding tissue.

How is Royal Peptides CBL-514 tested?

Every lot is tested by an independent third-party laboratory using HPLC for purity and mass spectrometry for identity. The lot-specific certificate of analysis is available on this page and ships with each order.

How should CBL-514 be stored?

Store lyophilized CBL-514 at −20 °C, sealed and protected from light and moisture. Allow the vial to reach room temperature before opening to prevent condensation.
